Percy Jackson: The Battle of the Labyrinth by Rick Riordan

SPOILER ALERT!

The fourth book starts with (of course) another set of monsters attacking Percy at school.  Percy successfully wards off an empousai, but their leader escapes.  He is helped to escape by Rachel Elizabeth Dare, a mortal who can see through the mist.

When Percy goes back to camp, he finds Quintus, a middle-aged demigod who is now the new swordsmanship teacher in the camp. In one of the mock battles Quintus arranged to prepare the demigods for battle, Annabeth and Percy stumble upon an entrance to the ancient labyrinth built by Daedalus.  The council of war is called, and Clarisse and Annabeth reveal their suspicions that Luke is trying to invade the camp using Princess Ariadne’s string which he will get from Daedalus himself.  As a result, Annabeth is asked to lead a quest to prevent this from happening.  Clarisse warns them, though, to be careful, as all who enter suffer a terrible fate: they go crazy.  She knows, as she saw Chris Rodriguez, a former camper, go crazy himself.

How can they succeed in finding the heart of the dangerous labyrinth, when all those who enter suffer from a crazed spell?  Why is there a coffin on the cover of the book? (hehe) Read Percy Jackson: The Battle of the Labyrinth and find out.
